The process is simple and convenient.

1) Call the office and book an appointment
2) Fill out and sign a Transfer of Record form
3) That’s it!

Regardless of whether if you are new to the neighbourhood, or looking for a new family dentist, or have an emergent dental issue, you have found a caring and friendly practitioner! Dr. Andy Shih and his staff invite you to join our family practice!

At your first visit, you’ll have a chance to meet the entire team at Komoka Family Dentistry, go on an office tour, and discuss any of your oral health concerns with Dr. Andy Shih. If you are eager enough, you can even have your dental cleaning!

If you have any previous dental records, such as recent treatments or dental x-rays, please bring it with you!

If you do not have any previous dental records, simply fill out the Transfer of Records form on the date of your appointment, so that we can request a copy on your behalf!

Our office is equipped to submit your dental claims electronically, on your behalf, on the day of the appointment for reimbursement. Our staff will also assist you, to the best of our abilities, in communicating with your insurance company related to dental claims and questions. Payment is required at the time of dental services. We gladly accept CREDIT CARDS, DEBIT CARDS, and CASH. Financial and payment plans can be coordinated in advance, by speaking with our office.



Your scheduled appointment has been reserved for you to see Dr. Andy Shih and our team. Please kindly notify us 48 hours in advance to reschedule your appointment. This courtesy allows our office to make arrangements to accommodate other patients who are seeking care.
